Service · Denials & appeals

Every denial is a lesson — and usually a recoverable claim

We work denials fast, appeal with the documentation reviewers look for, and fix the upstream cause so it does not repeat.

  • Denials worked within 48 hours of the EOB/ERA
  • Appeals built on clinical evidence and plan language
  • Monthly denial-reason trend report

Not every denial is the same

Sorting denials before anyone starts working them prevents wasted calls, missed appeal windows and incorrect patient balances.

Correctable

Examples include a missing tooth number or surface, an invalid subscriber ID, a missing attachment, or a claim that did not match the payer’s formatting rules. Fix the defect, document what changed and resubmit according to the payer’s process.

Appealable

These denials usually involve medical necessity, frequency exceptions, downgrades, alternate benefits, or “not supported by documentation.” Appeal only when the chart, images, perio charting, narrative and plan language support the service billed.

Non-covered

Plan exclusions, waiting periods, terminated coverage and benefits that were never available are not the same as clinical denials. Communicate clearly with the patient, follow your financial policy and use stronger eligibility and estimate workflows next time.

Reading the remittance

CARC and RARC codes, decoded

On electronic remittances, including X12 835 files, the denial reason is carried in standardized adjustment and remark codes.

Claim Adjustment Reason Codes, or CARCs, explain why a line was adjusted. Remittance Advice Remark Codes, or RARCs, add payer detail about what is missing, reduced or required next. Group codes help identify who is responsible for the balance: CO is a contractual obligation that usually should not be billed to the patient, PR is patient responsibility, and OA is an other adjustment category that needs review before posting.

A strong dental billing process does not simply post the denial and move on. Each adjustment should be tied to an action: correct and resubmit, appeal with documentation, transfer to patient responsibility, write off according to contract, or escalate for payer follow-up.

Common reasonLikely causeTypical fix
Frequency or limitation reachedThe service appears to fall inside the plan’s benefit window, or the payer has history from another office.Verify benefit history and plan frequency. Appeal only when clinical documentation and plan rules support an exception.
Missing or incomplete informationThe payer did not receive the attachment, narrative, tooth number, surface, quadrant, arch, periodontal charting, image, or other required detail.Send the missing item with the payer claim number. Add the requirement to the claim-scrub checklist for that payer and procedure.
Non-covered serviceThe service is excluded, subject to a waiting period, tied to an age limit, or not included under the patient’s plan.Confirm eligibility and plan language. Communicate patient responsibility clearly and improve pre-treatment estimates for similar cases.
Alternate benefit or downgrade appliedThe payer paid a lower benefit based on plan provisions, such as an alternate material or procedure benefit.Review the contract and explanation of benefits. Appeal when documentation supports the billed procedure and the plan allows reconsideration.
Coordination of benefits issueThe payer believes another plan is primary, or the patient’s COB information is missing or outdated.Confirm primary and secondary coverage, obtain updated COB information from the patient, and resubmit with the correct sequencing.
Timely filing denialThe claim or corrected claim was submitted after the payer’s filing limit, or the payer did not recognize the original submission.Check proof of timely submission, clearinghouse reports and payer acknowledgments. Appeal with documentation when available.
Prevention

Stopping repeat denials

  1. LogRecord every denial by payer, provider, location, CDT code, adjustment group, CARC or RARC when available, dollar amount and next action. Do not rely on memory or scattered notes.
  2. GroupReview denials monthly by root cause: eligibility, credentialing, coding, documentation, attachments, patient information, payer processing, coordination of benefits or timely filing.
  3. Fix upstreamUpdate verification scripts, claim-scrub rules, attachment checklists, narrative templates and clinical documentation prompts so the same issue does not repeat on the next batch.
  4. VerifyCompare the next month’s denial trends with the baseline. If the same payer, provider or procedure is still driving denials, escalate the workflow change and assign an owner.
