Know exactly what the plan will pay — before the patient sits down
Most dental denials and surprise patient balances start at the front desk. Our specialists complete eligibility and full benefit breakdowns 48–72 hours ahead of each visit and write them straight into your practice software.
Eligibility + full benefit breakdown, not just “active coverage”
Frequency, history and downgrade checks per procedure
Patient cost estimates your team can present with confidence
An “active” response confirms coverage exists. It does not confirm whether today’s crown, SRP, implant, appliance or oral surgery claim will be paid.
Electronic eligibility, often sent through the X12 270/271 transaction, is useful for a quick coverage check. But a basic dental 271 response may not include the details that drive payment: frequency limits, procedure history, missing-tooth clauses, alternate-benefit provisions, waiting periods, deductibles, remaining maximums or prior-authorization rules.
Those gaps show up later as denied claims, avoidable write-offs, delayed treatment acceptance and uncomfortable patient balance conversations.
A full benefit breakdown closes the gap. Dental Billing Community verifies the plan-level rules and the procedure-level rules for the treatment actually scheduled, then flags what needs a pre-treatment estimate, narrative, X-ray, medical benefit check or payer follow-up before the patient arrives.
What we verify
A complete benefit breakdown, every time
Documented in your practice management system so the front desk, clinical team and billing team work from the same information.
Plan and eligibility
Subscriber and dependent status, effective date, termination date when available, group information, payer contact details and plan type, including PPO, DHMO, indemnity, Medicaid or CHIP, and Medicare Advantage dental when applicable.
Deductibles and maximums
Individual and family deductibles, amounts met to date, annual maximum remaining, orthodontic lifetime maximum when applicable, and whether preventive, diagnostic or other categories reduce the annual maximum.
Frequencies and history
Last service dates for exams, prophylaxis, periodontal maintenance, bitewings, full-mouth series or panoramic imaging, fluoride, sealants and scaling and root planing so the team can avoid scheduling too early.
Limitations and exclusions
Waiting periods, missing-tooth clauses, age limits, replacement rules for crowns, bridges and dentures, implant limitations, periodontal limitations and documentation requirements that could affect reimbursement.
Downgrades and alternate benefits
Posterior composite downgrades, crown material downgrades, implant alternate-benefit provisions and other payer rules that may change the patient estimate before treatment is accepted.
Coordination of benefits
Primary and secondary order, including common dependent-child rules such as the birthday rule, non-duplication provisions, carve-outs and whether medical benefits should also be checked for oral surgery or related services.
Verified benefits are written into the patient record so every team member sees the same numbers.
How it works
Our verification workflow
Built around your schedule, your payer mix and the procedures patients are actually booked for.
Schedule syncWe review upcoming appointments from your practice software, typically focusing on the next 3–5 business days and any high-value treatment visits that need earlier attention.
Electronic firstWe use electronic eligibility and payer portals where available because they are faster and create a record. When the portal does not answer the clinical billing question, we escalate to payer research or a call.
Procedure-level checkScheduled and treatment-planned CDT codes are reviewed against plan rules, frequencies, history, age limits, replacement clauses and documentation requirements. CDT updates take effect each January 1, so codes and payer edits must be checked against the current year.
Estimate and flagsWe note estimated patient responsibility and practical flags, such as pre-treatment estimate recommended, prior authorization required, frequency conflict, waiting period, downgrade risk or missing history.
Same-day exceptionsWalk-ins, emergencies and schedule changes are verified the same day on request, with clear notes when the payer cannot provide complete information before treatment.

Quick verification checklist for dental teams
If your team verifies in-house today, this is the minimum information to capture before quoting benefits to a patient.
Area
What to confirm
Why it matters
Coverage
Active status, effective date, plan type, network status and payer contact path.
Prevents basic eligibility and out-of-network surprises.
Money
Deductible, amount met, annual maximum, remaining maximum and category percentages.
Improves patient estimates and collection timing.
History
Last dates for exams, cleanings, perio maintenance, X-rays, fluoride, sealants and SRP.
Reduces denials tied to frequency limits.
Major work
Replacement periods, missing-tooth clauses, downgrades, implant rules and documentation.
Protects crowns, bridges, dentures, implants and other high-dollar treatment.
Authorization
Pre-treatment estimate, prior authorization or referral requirements.
Helps the team collect needed records before the visit.
COB
Primary/secondary order, non-duplication language and carve-outs.
Prevents incorrect billing order and overstated estimates.
Medical crossover
When dental verification should include medical benefits
Some dental practices lose time because the team checks only the dental plan when the case may also involve medical coverage.
For oral surgery, trauma, biopsies, TMD-related services, sleep apnea oral appliance therapy and some medically necessary dental services, the verification process may need to include medical benefits, prior-authorization rules, diagnosis requirements, network status and claim routing.
For obstructive sleep apnea, the ICD-10-CM diagnosis code commonly used is G47.33, which means obstructive sleep apnea. Medicare coverage for a custom oral appliance generally uses HCPCS E0486 for a custom-fabricated mandibular advancement device, but Medicare requires a physician diagnosis, applicable LCD criteria, and DMEPOS enrollment. Practices should verify the current LCD and payer policy before scheduling or billing.
For Medicare dental services that are inextricably linked to a covered medical service, claims may require an appropriate diagnosis code and the correct modifier, such as KX when coverage requirements are met or GY when the item or service is expected to be non-covered. Confirm the current Medicare guidance and payer instructions before submission.

Common questions
Insurance verification FAQ
Do you call the payer or use portals?
Both. We start with electronic eligibility and payer portals because they are faster and usually leave an audit trail. We call or escalate when the portal does not answer procedure-level questions, such as replacement history, alternate benefits or authorization rules.
Can you verify medical insurance for oral surgery or sleep appliances?
Yes. For procedures that may involve medical benefits, including some oral surgery, biopsies, trauma, TMD-related care and oral appliance therapy, we verify medical benefits, prior-authorization rules, diagnosis requirements and network status alongside dental coverage.
How do you handle patients with two dental plans?
We verify both plans, identify the likely primary and secondary order using the plans’ coordination-of-benefits rules, and document non-duplication or carve-out language that may limit secondary payment.
Where do the results go?
Results are entered into your practice management system in the appropriate benefits, insurance and appointment-note areas, so the front desk, treatment coordinator, clinical team and billing team can see the same information in their normal workflow.
Can you work with our existing front desk?
Yes. Many practices use us to extend the front desk rather than replace it. We can handle routine verification, high-dollar treatment verification, medical benefit checks, or cleanup projects when the schedule is too full for the in-house team.
